Another update: Since the last post, he had a 2nd lupron injection and then a deslorelin implant when it became available in late February. This helped with the itching plus I did an elimination diet to find foods that caused problems.
By early April, his fasting BG was lower than normal so, reluctantly, at the vet's suggestion, he started back on prednisolone. Initially he was taking the pediapred from the vet's office which he did not do well with (large bleeding MCT) - must have been a response to whatever pediapred is flavored with.
Switched him to the compounded prednisolone and he was taking .015ml of 3mg/ml. Unfortunately, he very quickly regained the abdominal fat and became lethargic with a some hind end weakness. When I was looking for more information about prednisolone side effects, I found that it can cause Cushings Syndrome (excess cortisol) which accounts for the symptoms I was seeing.

Took him to the vet to talk about backing him off the prednisolone - had an xray done to discover a significant amount of fat in the chest cavity as well, which was causing discomfort and breathing issues. He'll be completely off pred in about 10 more days - I am also increasing the diazoxide at the same time as reducing the pred.
Poor kid is not doing very well - I'm feeding him multiple very small meals and he eats them well. His respiration when sleeping is steady and not labored but I am concerned. Although he dropped the fat fairly quickly when I took him off pred last year, I do fear it won't be soon enough.
